How Do I Resubmit Insurance Enrollment to Medicare?

Medicare Beneficiaries

You are eligible for medicare benefits when you are sixty five years or older. You can also be eligible for medicare benefits if you have a disability and are younger than sixty five. You are eligible to submit an insurance enrollment form to Medicare three months after you turn sixty five. This initial enrollment period will last for seven months. If you do not enroll during this period or if you were denied during this period, you can resubmit the insurance enrollment form for Medicare during the general enrollment period. The general enrollment period usually lasts from January 1st to March 31st. You can submit a Medicare 855R application if you wish to reassign your Medicare benefits.

Health Care Providers

If you are a health care provider who wishes to resubmit an insurance enrollment form to Medicare, you can submit an electronic form through the Medicare Web site. You can also submit a paper application to the Centers for Medicare and Medicaid Services.

Physician Practitioners

As a physician practitioner, you can submit the Medicare 855l application if you wish to change your enrollment information. This must be filed with a Medicare contractor.

Suppliers of Medical Equipment

If you supply medical equipment to others and wish to resubmit an insurance enrollment application, then you should send in a Medicare 855S application to a Medicare contractor.

Hospitals, Home Health Agencies, and Community Mental Health Agencies

If you work with a hospital, a home health agency, or a community mental health agency, you cal also submit or resubmit a Medicare insurance enrollment application. You should send in the 855A application to a Medicare contractor.
