You are eligible for medicare benefits when you are sixty five years or older. You can also be eligible for medicare benefits if you have a disability and are younger than sixty five. As a physician practitioner, you can submit the Medicare 855l application if you wish to change your enrollment information. This must be filed with a Medicare contractor.
Suppliers of Medical Equipment
If you supply medical equipment to others and wish to resubmit an insurance enrollment application, then you should send in a Medicare 855S application to a Medicare contractor.
Hospitals, Home Health Agencies, and Community Mental Health Agencies
If you work with a hospital, a home health agency, or a community mental health agency, you cal also submit or resubmit a Medicare insurance enrollment application. You should send in the 855A application to a Medicare contractor.
